| 内容記述 | がん患者の配偶者を対象とした看護の研究は先立ってしばしば実施されていることから，本研究は両者ががんの夫婦に着目し，両者ががんである側面からみた夫婦の関係性の様相を記述することを目的とした．研究方法は，夫婦一単位を対象としたライフストーリー・インタビューによりデータを収集し，分析は，夫婦ごとのデータ全体を精読し，意味を縮約して1 つの本質的なテーマを生成し，これを最も表現している対象者のそのままの語りを用いて記述した．倫理的配慮として，香川県立保健医療大学倫理審査委員会の審査を受け，承認を得た．結果，対象は2 組の夫婦（A・B）であり，平均年齢は67 歳，がん種は，夫はA・B とも前立腺，妻A・B はそれぞれ食道，膀胱であった．そして本質的なテーマは，＜同時期にがんに罹患した夫婦だからこそお互いを思いやり2人でともに生きてゆく＞＜がんへの向き合い方を自然なかたちで尊重し合う＞があった．両者ががんである夫婦は，これまでに互いに知り得た相手の思考や行動，性格などを尊重し，夫婦でがんとともに生きる経験により，相手をより多くの側面から理解して新たな夫婦の関係性を築き，絆を深めていた．両者ががんである夫婦への看護は，夫婦の関係性は，がんの診断期，治療期，進行期，終末期などの状況において変化し得るため，夫と妻がどういったがんの状況があっての関係性であるかを把握し，個としての夫・妻にも関心を払い，夫婦の関係性に関連する対処しきれない事柄や整理できない心理面などの問題を汲み取り，解決に向かうよう支援していくことが重要である． The purpose of this study was to describe the relationship aspects of couples who both had cancer. The data was collected through life-story interviews with each couple as a unit, and the analysis was conducted by reading the entire data for each couple closely, contracting the meanings to generate essential themes, and describing them using the verbatim narratives of the subjects who best expressed them. As an ethical consideration, the study was reviewed and approved by the Kagawa Prefectural University of Health Sciences Ethical Review Committee. The results showed that the subjects consisted of two couples (A and B), the average age was 67 years, and the cancer types were those in prostate for both husbands A and B, and in esophagus and bladder for wives A and B, respectively. The essential themes of the study include; <being considerate of each other and living together because we are a couple who were both diagnosed with cancer at the same time> and <respecting each other's way of dealing with cancer in a natural way>. The couples who both had cancer respected their spouse's thoughts, behaviors, and personality that they had learned about each other, and through their experience of living with cancer as a couple, they understood their spouse in more aspects, built new marital relationships, and strengthened their bond. Therefore, it is important to understand the relationship between husband and wife, to pay attention to the husband and wife as individuals, and to support them in finding solutions to problems such as those that cannot be dealt with or psychological problems that cannot be sorted out. |
